The Mukh Mantri Sehat Bima Yojana (MMSBY) is a comprehensive health insurance scheme launched by the Government of Punjab in collaboration with the Ayushman Bharat initiative. Launched on August 20, 2019, this scheme is designed to provide quality healthcare to economically vulnerable families in the state of Punjab.
Under MMSBY, eligible families are provided with a cashless health insurance cover of up to ₹5 lakh per year. The coverage includes secondary and tertiary care treatments such as hospitalization expenses, surgery, pre- and post-hospitalization costs, and treatment for newborns. The scheme offers financial protection for over 1,579 medical treatment packages ranging from routine hospital care to complex surgeries.
The scheme is aimed at 1.79 crore families in Punjab, with 22.12 lakh families being fully covered by the state and the rest benefiting from the central-state funding in a 60:40 ratio. The Punjab Government bears the full premium cost for eligible families. Beneficiaries can access treatment at empaneled public and private hospitals across Punjab and even in other parts of India, ensuring a wide reach.

- •Residents of Punjab.
- •Families listed in SECC 2011 data.
- •Ration Card (NFSA) holders.
- •J-form holder farmers.
- •Small and marginal farmers.
- •Registered small traders with the Excise & Taxation Department.
- •Registered construction workers.
- •Yellow Card journalists.
Documents Needed
- •Aadhaar Card: For identity verification.
- •Ration Card: To confirm family details.
- •Family Declaration Form: Required if you do not have a ration card; must be signed and stamped by a Sarpanch, Nambardar, or Municipal Councillor.
- •Income Certificate: To verify economic status.
- •Construction Worker Registration Card: If applicable.
Application Process
- •Eligibility Verification: Ensure you are listed in the SECC 2011 data, hold a valid NFSA Ration Card, or belong to other specified categories such as small traders, construction workers, or J-form holding farmers.
- •E-Card Generation: Visit the nearest Common Service Center (CSC) or Sewa Kendra to get your e-card.
- •Documentation Submission: Submit the required documents (listed below) to the center.
- •Payment: Pay a nominal fee of ₹30 for the e-card issuance.
- •Receive E-Card: Collect your Mukh Mantri Sehat Bima Yojana e-card, which you can use to avail cashless treatment at empaneled hospitals
